The hour

Eight patients a day. Not eight before lunch.

Most hygienists we meet were trained to do one kind of hygiene and paid to do another. Here the book is built for the first kind. Adult recall and perio maintenance get 60 minutes. Kids get 45. New patients get 90.

Joy, our office manager, builds the hygiene schedule herself and never double-books a hygiene chair to fill a hole. When a patient cancels, the hour is yours: charting, a sterilization hand, a call back to a nervous patient, or a coffee.

“I get to finish a sentence with my patients.”

Denise Holloway, RDH · lead hygienist, here since 2016
The Mill from the street at first light: a low red-brick building with tall windows under a big oak on Oakmere Avenue.

Our story

A florist's old building, a dentist who wanted the opposite of a chain.

Dr. Anna Castellow spent six years in two corporate offices in the Triangle. She liked the dentistry. She did not like 40-minute blocks, a hygiene department measured by what it converted, or a team that turned over so fast she stopped learning names.

In September 2016 she opened the Mill in a low red-brick building on Oakmere Avenue, a few minutes' walk from Five Points. She kept the brick, the tall windows and the oak floor, and made everything clinical brand new.

Ten years on: about 2,400 patients, eight people instead of four, whole families on one schedule. A grandmother at 8:00, her daughter at 9:00, the grandkids after school.

“Denise sat beside me my whole first month. Four years later I still bring her the strange charts.”

Joy puts a record on in the waiting room before the first patient, morning light on the brick and the oak floor.

A day at the Mill

What Tuesday looks like.

  1. Light on the oak floor. Joy puts a record on, quietly. First patient.

  2. A grandmother, then her daughter at 9:30. Same family, same chair.

  3. Dr. Duarte comes to your room for an exam.

  4. Lunch. A full hour, one table, everyone at it.

  5. After school. A six-year-old picks the ceiling photo to stare at.

  6. Done. Or 6:00, on your rotating Tuesday.

  7. Friday

    Off.

Every treatment room has a window or skylight, and a framed photo of a Raleigh place on the ceiling above the chair: the Rose Garden, a greenway bridge, the Fair wheel at night.

The whole team laughing around one oak table at lunch, a loaf of something homemade in the middle.

12:30. One table, everybody at it.

See the building and the people before you ever walk in.

62 seconds. The team speaks in it, so sound on if you can.

The honest part

What might not suit you.

Better you hear it here than in month three.

  • We're small.Eight people. When someone's out, you might room your own patient or pick up a phone.
  • No ladder. There's no regional director job to climb toward. Growth here means wider work.
  • No production bonus.A top producer at a chain can out-earn you in a big month. We put that money in the hourly rate instead.
  • No sign-on bonus. We'd rather pay you well in year three than once in week one.
  • Kids are kids.Tears, wiggles, a parent standing in the room. If that wears you out, this is the wrong chair.
  • One later day.A Tuesday to 6:00, on rotation.
  • Parking is tight. An old building, a small lot, the street. On a busy Friday you'd circle the block.
  • You chart your own.No dedicated hygiene assistant. Marcus and Hannah help when they can.
